Description

IMMEDIATE AND ON-GOING OPENINGS FOR LIFE SKILLS COACHES (full-time and part-time)
Immediate full and part-time positions available to work with our clients in our Community Based and Residential programs, working in the community, volunteer and vocational settings, and in their homes (where personal care and light housekeeping duties may be required). Various opportunities are available in our Community Based Services, Residential, Independent Living, In-Home Supports, Family-Directed and Employment Services programs. Hours range from Monday through Friday in some programs, to nights, holidays or weekends, to per diem or relief hours.

Description

Title Clinical Life Skills Coach

CLINICAL LIFE SKILLS COACH / DIRECT SUPPORT PROFESSIONALS
Several immediate full-time openings to professionally support individuals with complex needs receiving services in a Clinical Home to achieve the highest level of independence in all settings. 

Bachelor’s degree preferred, but not required. Minimum of 1 year experience in human services required; 3 years’ experience preferred. Applicants must also pass criminal, motor vehicle, and Bureau of Elderly and Adult Services background checks that meet agency standards, and possess a valid driver’s license and access to a safe, operating and insured vehicle during scheduled working hours. Prior completion of management of aggressive behavior (MOAB) training part 1 and part 2 and required state-designated training through Relias Learning or equivalent is preferred, but we’ll provide the necessary training to qualified candidates. The Moore Center also offers a wide variety of additional training in human services through Relias Learning to all Life Skills Coaches.

Title Life Skills Coach

Description

We have an immediate part-time opening for a Life Skills Coach to provide care for a 16-year-old child in his home. Duties include assisting with getting child off of the school bus as well as participation in leisure and community-based activities. Responsible for implementing and documenting client therapies and interventions as indicated in the Individual Service Plan.  Hours are Monday through Friday, 2:30pm – 5:30pm.

Support Needed for Craig

CraigThe Moore Center supports families who care for loved ones with intellectual and developmental disabilities. 

This request comes from a loving mother in Bedford, NH who’s looking to hire a caring person to help support her 22 year old son, Craig, who was born with a rare chromosome anomaly. 

It’s a 30 hour paid position, including health/dental insurance and vacation time. It’s a rewarding position for an LNA or a mom looking for a job that makes a real difference. Formal personal care training is provided, in addition to hands-on training by Craig’s mom, Laura.

Here’s a message from Craig’s mom:

Most people don’t understand the day to day life of a mom who cares for her son with a disability.  Although every situation is different, I’d like to tell you about mine.

My son Craig was born with a rare chromosome anomaly and he can’t walk or speak.  He can, however, express happiness, appreciation and let us know when he’s unhappy too.

Although he uses a wheelchair, he is still able to help me in the garden. He also enjoys craft activities, going for walks, live music, and visiting seniors at our local nursing home (where he’s a bit of a celebrity!). He requires help with feeding, dressing and other personal care. I focus on what Craig CAN do and I celebrate all of his accomplishments, no matter how small. 

As much as I’d like to, I can’t be available 7 days a week, 24 hours a day. My two other sons have provided additional support but now they are going off to college in September and I would love some help.

I am offering a 30 hour per week paid position, including health insurance and vacation time, to a compassionate person who wants to work to help make each day brighter for Craig. I can teach you what makes him happy and the other parts of a challenging, but satisfying, job. And I will be forever grateful for all you do for Craig.

Title Manager of Individual Supports

An exciting opportunity for a Manager of Individual Supports to provide program management and oversight for a group of clients in an adult day program with varying and individualized needs, many of which having behavioral implications.

Essential Responsibilities:

  • Hire, train and supervise all program staff
  • Assess and match staff with clients that result in high quality of service delivery
  • Coordinate staff scheduling for program coverage and training
  • Collaborate with and support other IDS managers, particularly regarding shared responsibilities, e.g. staffing, transportation, and van care
  • Ensure that program operations, budgetary needs, billing units, documentation, client and staff schedules and program activities are in compliance with state, federal and Agency regulations and policies
  • Coordinate and implement weekly schedules and program activities for clients
  • Track and record program recreational expenses
  • Act as designated trainer for staff in collaboration with Behavior Specialist on behavior plans for clients as needed
  • Perform annual Performance Development Plans and monitor goals and staff development on an ongoing basis
  • Work with IDS team on ongoing program and curriculum development
  • Attend individual service agreement and other client meetings as part of an interdisciplinary team
  • Develop and write measurable and achievable day services goals for clients
  • Be knowledgeable and up to date on important client information to ensure quality services are being delivered
  • Teamwork, regular attendance in the office, and face-to-face interaction are essential job functions
  • Complete other duties and responsibilities as assigned

 

Qualifications:

  • Minimum of a Bachelor’s degree in Human Services (or related field) required
  • 3 years’ experience in the field of human services (developmental services preferred)
  • 1 year management experience required (including hiring, training, and supervising staff) or on-the-job management training
  • Experience working with people who have behavioral and/or forensic/high risk challenges required
  • Valid driver’s license and a reliable, insured vehicle
